Mental Health Forms

Mental Health records are available for the Qualified Mental Health Professional to complete as well as a Psychiatrist. The current forms include:
• Detainee in Observation
• Non-Contact Professional Activity
• Clinical Contact
• Screening Interview
• Detainee in Segregation
• Clinical Contact Psychiatry

Each form has the optional workflow to “Refer to Medical” for further evaluation. A follow-up date may also be set to generate a task reminder for the QMHP.
